Using the Patient Portal

How to access your Patient Portal

  1. Log in to the Patient Portal here. (Or hover under the ‘Patient Resources’ tab above and select ‘Access Patient Portal’!)
  2. If you have a Patient Portal Account enter your username and password to ‘Login.’

Otherwise, simply fill out the fields to start your enrollment. If you do not have an email, please call our office to assist you.

How to send a secure message to your physician

  1. On the homepage, navigate to the ‘Inbox’ and click the blue ‘Compose an Email’ link.
  2. Make a Category selection: e.g. ‘Billing Questions’
  3. Select your Physician from the pull-down menu.
  4. Enter a ‘Subject’ and type your ‘Message’
  5. When finished, click ‘Submit’ button.
  6. You should receive an email alert when a reply to your message appears in your ‘Inbox’

How to request medication refills

  1. On the homepage, navigate to the ‘Medications’ grid.
  2. To request a refill, click on the blue ‘refill’ link of the medication.
  3. Confirm the correct medication and pharmacy are selected.
  4. From the pull-down menu, select ‘Renewal of Ongoing Medications.’
  5. Select your Physician from the pull-down menu.
  6. Add comments and click ‘Submit’ button.

How to schedule an appointment

  1. On the homepage, navigate to the ‘Upcoming Appointments’ grid.
  2. To request an appointment, click on the blue ‘Schedule an Appointment’ link.
  3. Select your Provider and ‘Portal Appointment’ from the pull-down menus.
  4. Select the location you wish to visit.
  5. Enter a short note as a reason for appointment and populate your available date(s) and time(s).
  6. When finished, click ‘Submit’ button.
